Accident & incident history

2005-07-04 Accident · Weston, WV Damage: Destroyed NTSB · IAD05CA092
Reported aircraft: Callahan Rotorway Exec 90 — a different aircraft that carried the N4359N registration before this airframe.
Probable cause: A loss of engine power for undetermined reasons during an attempted power recovery. A factor in the accident was the pilot's failure to maintain rotor rpm, which resulted in a hard landing and subsequent post crash fire.
1999-06-21 Accident · Lutz, FL Damage: Substantial Serious injury NTSB · MIA99LA186
Reported aircraft: Robert R. Callahan Exec 90 — a different aircraft that carried the N4359N registration before this airframe.
Probable cause: The excessive cyclic input by the non-qualified passenger during the autorotative landing following a reported loss of engine power. A factor in the accident was the inadequate preflight of the helicopter by the unqualified pilot for his failure to detect corrosion at each ignition pack during the helicopter preflight, resulting in the loss of engine power.
