Accident & incident history

2004-09-19 Accident · Memphis, TN Damage: Substantial NTSB · DCA04MA082
Reported aircraft: Boeing MD-11 — a different aircraft that carried the N601FE registration before this airframe.
Probable cause: The pilot's over-rotation during a go-around maneuver initiated because of a bounced landing. The go-around maneuver was initiated at a low speed and high pitch angle, and after reverse thrust was selected, contrary to Boeing and FedEx training guidance.
2002-06-02 Accident · Subic Bay Damage: Substantial NTSB · DCA02MA042
Reported aircraft: McDonnell Douglas MD-11F
Probable cause: The failure of the lower outboard nut and bolt assembly of the left inboard flap's outboard hinge that resulted from stress corrosion cracking and fatigue. Factors contributing to the accident were the inadequate materials specification of the bolt, and inadequate inspection requirements for the assembly.
