Probable cause: FAILURE OF THE TOWER CAB SUPERVISOR TO ASSURE THAT COORDINATION BETWEEN THE SOUTH LOCAL CONTROLLER AND NORTH LOCAL CONTROLLER HAD BEEN COMPLETED. A FACTOR IS THE FAILURE OF FAA TOWER MANAGEMENT TO INSURE THERE WERE PERSONNEL MANNING THE CAB COORDINATOR POSITION.
