Reported aircraft: McDonnell Douglas 369FF — a different aircraft that carried the N911VC registration before this airframe.
Probable cause: The fatigue failure of the tail rotor blade root fitting due to a manufacturing defect which resulted in the separation of the tail rotor system and loss of tail rotor control. A contributing factor to the accident were the trees that the helicopter struck during the autorotation.
1993-06-30 Accident · Agoura Hills, CA Damage: Destroyed Minor injury NTSB · LAX93GA270
Reported aircraft: Bell 206L-3 — a different aircraft that carried the N911VC registration before this airframe.
Probable cause: The improper configuration of the aerial ignition device by the crew chief and an inadvertent activation of the unit by the pilot. The improper installation and configuration of the unit by maintenance personnel, inadequate crew coordination, and insufficient aviation unit operational standards were factors in this accident.
