Accident & incident history

2013-03-18 Accident · Panama City, FL Damage: Substantial Minor injury NTSB · ERA13LA174
Reported aircraft: Piper PA-25-235
Probable cause: A loss of preload to the crankshaft gear attachment bolt, resulting in rotation of the crankshaft gear and a subsequent total loss of engine power. The root cause for the loss of preload could not be determined during a postaccident examination of the components.
2005-12-18 Accident · Opa-Locka, FL Damage: Substantial Minor injury NTSB · MIA06LA033
Reported aircraft: Piper PA-25-235
Probable cause: The failure of the pilot-in-command to maintain airspeed following a reported loss of engine power, resulting in a stall/mush, uncontrolled descent, and in-flight collision with terrain. Factors in the accident were the pilot's intentional operation of the airplane with known deficiencies (inoperative No. 3 banner release), and inadequate installation of the banner tow-release hook assembly by company maintenance personnel.
1986-07-30 Accident · Friedens, PA Damage: Destroyed Serious injury NTSB · NYC86FGM05
Reported aircraft: Bomberger Vans RV-4 — a different aircraft that carried the N86AB registration before this airframe.
