Accident & incident history

2002-11-27 Accident · Woodland, CA Damage: Substantial Serious injury NTSB · LAX03LA041
Reported aircraft: Clyde K Carrier Team 1400Z — a different aircraft that carried the N9190U registration before this airframe.
Probable cause: the builder/pilot's improper modification of wing fuel caps resulting in the failure of the vent system to adequately compensate for the pressure differential between the fuel tank and the external low pressure area of the wing. This led to the partial loss of engine power in cruise flight due to fuel starvation, and a forced landing.
1991-07-31 Accident · Hillsboro, OR Damage: Destroyed NTSB · SEA91LA197
Reported aircraft: Cessna 150M — a different aircraft that carried the N9190U registration before this airframe.
Probable cause: FAILURE OF THE PILOT TO REFUEL RESULTING IN FUEL EXHAUSTION.
